The output is repeatable: print the same model a second time and you get the same piece. On the hand side, there's no model to print. An artisan holds the reference photos, builds a small clay body from scratch, refines the face and posture by hand, then paints the surface feature by feature. The starting point and the finish are both human, which is why two figures from the same studio will never come out identical — even for the same subject.

What should I actually look for when choosing?

Ask three things. First, who made it — if the answer is "a printer," you're looking at the manufactured side. Second, how is it finished — hand-painted polymer clay shows visible detail and texture that catch light; a printed surface tends to read smooth and uniform. Third, does it say so plainly — "sculpted by hand, one at a time, from your photos" is the honest phrase worth keeping in mind. When it matters to you, that's the line that separates a keepsake from a mass-produced token.

Is the handmade option just slower, or is there a real trade-off?

It's both. You're trading speed for the kind of detail a template can't give you. For a generic object, that trade is rarely worth it.
